STAA Fall 2008 Soccer Sign-up

Grades K4-8th  

 

If your child would like to play soccer, please complete this form and return it to school to the attention of Jeff Demeyers by June 11th.  After June 11th, contact Jeff  by phone or e-mail.  The sign up cutoff date is August 31st.  If registration is late, acceptance to the soccer program will be at the discretion of the Athletic Association. Team commitments must be submitted to the league by September 1st. ·         The athletic fee is $70, OR 2 boxes of candy bars and $20 for each participant’s first sport, then $20 for each subsequent sport with a $180 maximum per family.  The candy bars will be available during fall and winter sports seasons only.   Please indicate which method you would like to use on the sign-up form.

·        The operating costs of the sports program (league fees, uniforms, equipment, etc.) are primarily funded by revenue generated from hosting games and tournaments during the volleyball and basketball seasons. For this reason, all sports participants are asked to volunteer for gym time. For each soccer season (fall and spring), it is a 2 shift requirement for one player, and 3 shifts for more than one player. Families that do not serve the appropriate amount of volunteer time by the end of basketball season will be invoiced $40 per shift in March 2009. ·        The Athletic Association is asked to commit the number of teams it expects to field to the league in advance of each sport season. The school must pay a forfeiture fee and risk the possibility of expulsion if we do not hold to this commitment. We will consider the signing of this form to be a commitment to participate in the soccer program. If you wish too withdraw after September 8th, 2008, there will be a $25 fee. Failure to pay this fee may result in suspension from sports.

·          Participating children in grades 5 through 8 are required to have a Physical Examination Form signed by their doctor once every 2 years.  Practice begins on August 15th and physicals must be completed and returned to Tom Dineen at this time or you will be ineligible to participate.  Tom Dineen can be contacted at 294-4770 or tdineen1@wi.rr.com   Blank forms are available on the St. Thomas Aquinas website at www.thomasaquinasacademy.com
